Each student needs to have thias form completed by a parent/guardian & bring ~$10 for their dinner (more if they want dessert, etc...) The bus/transportation will leave at 5:30pm from the front of the church near the office and arrive back when the event is complete (around 8:30pm). Your student agrees to be on their best behavior and listen and comply with all direction given to them by the adult chaperones supervising this trip. PARENTS/GUARDIANS: Please indicate your approval at the bottom of this form which will serve as your signature. Your contact number will serve as their emergency contact.
